RELEASE OF LIABILITY FORM
I understand and agree that the Discover The Bible
Lands! October 2008 In the Footsteps of Paul Tour,
organized in conjunction with First Presbyterian Church of
Bonita, Florida, and MTS TRAVEL, of which I am a participant,
involves certain risks, and that regardless of the
precautions taken by the organizations involved, bodily
injuries may occur.
Specific risks or hazards include, but are not limited
to, injuries received during travel to or from destinations,
physical activities such as hiking, swimming, and the like,
in which I may choose to participate in during the course
of the Tour. I acknowledge that I am to practice personal
responsibility for my well-being during my participation in
the Tour.
I further understand and agree that Discover The Bible
Lands! cannot be expected to control all of the risks associated
with a Tour of this nature, but that a response may
be necessary in the event of an accident or emergency.
Therefore, I hereby give my consent to emergency medical
treatment that may be required by me during the Tour,
with the understanding that the cost of any such treatment
will be my responsibility. I understand that reasonable attempts
will be made to contact my “emergency contact” for
permission and instructions before treatment, unless it is
an immediate, critical situation.
Discover The Bible Lands! does not carry group medical
or accident insurance for tour participants. I understand
that I am responsible for my own insurance. I acknowledge
that I have been informed of my responsibility to review my
personal insurance needs, and to obtain such insurance
as I deem appropriate.
Knowing this information, and in consideration of
my desire to participate in the Tour, I hereby expressly,
voluntarily and knowingly assume the potential risks and
release Discover The Bible Lands!, its representatives,
advisors, agents, members, assigns, and founder, as well
as their heirs, legal representatives, and assigns from any
and all claims and causes of action for property damage,
personal injury or death sustained by me arising out of any
travel or activity conducted by or under the auspices of
Discover The Bible Lands!, including defense costs and attorney’s
fees, caused by risks associated with the October
2008 Tour and/or negligence of Discover The Bible Lands!
PERSONAL COMMITMENT
By participating in Discover The Bible Lands! Tours, I understand that I am expected to follow Biblical guidelines of
Christian conduct, bringing honor to Christ by my behavior and courtesy to other Tour participants and those we meet
throughout the journey. I commit to show love and respect, and to esteem others higher than myself.
I have read this Release Form and Personal Commitment and willingly sign this Release for the consideration
expressed and with a full understanding of its purpose. I am eighteen (18) years of age or older and am otherwise
competent to execute this agreement, or I am the legal guardian of a participant who is under age eighteen (18).
I understand that this Release is binding upon myself, as well as upon my heirs, executors, administrators, and
assigns.
